Insurance ID Cards

Many of you still have not received insurance cards from our new providers Aetna Health, Guardian Dental, and VSP Vision. If you have not received your insurance cards you can access them online.

Below are are the links to create an account for each insurance provider. If you have any questions please contact Kenny Harvey, [email protected].

Aetna Health: https://www.aetna.com/AccountManagerV3/register/selfService/registerMember

1) Under "Sign Up Using My" select "Social Security number"

2) Complete steps 1-4

3) Login to your Aetna account

4) Print ID cards on the home page by clicking "View Member ID Cards"

Guardian Dental:

1) Select "Member"

2) Click "I Agree"

3) Fill in required information, Use Group ID "00404556"

4) Login to your Guardian account

5) Print ID cards on the home page by clicking "ID Cards *NEW*"

VSP Vision:

https://www.vsp.com/create-account

1) Under "SSN or Member ID" use the last 4 numbers of you social security number

2) Login to your VSP account 3) Print ID cards on the home page by clicking "View Member ID Card"
